PRIMARY INTENDED OUTCOME
Human drug compounding is an important branch of pharmacy practice that
creates medications tailored to the health needs of specifc patients. Compounded
drugs improve and save lives by providing medicines that may be unavailable or
not commercially sold in a particular formulation. However, compounded drugs
are generally not required to meet federal approval and can be at risk
for contamination.
Charles River provides a range of microbiological testing services and equipment
to support drug discovery, development, and manufacturing that can be tailored
to a facility’s specifc needs and ensure patient safety.
THE CHALLENGE
In 2012, an outbreak of fungal meningitis caused numerous cases of serious
illness and more than 60 deaths, according to the CDC. The public health tragedy
resulted from contaminated injectable steroids that were produced by the
New England Compounding Center (NECC). Confusion over state and federal
oversight was called out as a contributing factor. The NECC was also found to be
acting, erroneously, as a drug manufacturer, which would be subject to diferent
regulations than a compounding pharmacy.
Compounding pharmacies are now divided into two sectors. Designated 503A
facilities are limited to dispensing small amounts of drugs and must follow USP
and state pharmacy guidelines. Larger 503B pharmacies are allowed to use bigger
batches and must validate every process with current good manufacturing
practices (cGMPs). These 503B requirements include more frequent environmental
monitoring, on-site inspections, and submitting products for testing, which can
result in longer lead times.

U.S. Hospitals Can Take to Combat Antibiotic Overuse and Reduce Use by 11.4%
THE CHALLENGE:
Though antibiotics have transformed the practice of medicine, the Centers
for Disease Control and Prevention (CDC) warn that 20-50% of all antibiotics
prescribed in U.S. acute care hospitals are either unnecessary or inappropriate.
The misuse of antibiotics has contributed to the growing problem of antibiotic
resistance, which has become one of the most serious threats to public health.1
More than two million people are infected with antibiotic-resistant organisms
each year, resulting in approximately 23,000 deaths annually.2 According to
the World Health Organization (WHO), antimicrobial resistance threatens the
effective prevention and treatment of an ever-increasing range of infections
caused by bacteria, parasites, viruses and fungi.3
In response, the Joint Commission published a new Medication Management
(MM) standard for hospitals, critical access hospitals and nursing care centers,
which became effective January 1, 2017, emphasizing the need to reduce
use of inappropriate antimicrobials in all health care settings, and requiring
hospitals to have an antimicrobial stewardship program that is based on
current scientific literature.4
The Centers for Medicare and Medicaid Services (CMS) proposed new rule
(June 2016) requires hospitals to reduce antibiotic use in order to participate
in Medicare or Medicaid and states that hospitals must appoint designated,
qualified leaders for those antimicrobial stewardship programs.5,6
To support hospital and health systems in meeting the Joint Commission
standard and the CMS proposed rule, Comprehensive Pharmacy Services
(CPS) launched the first of its kind Antimicrobial Stewardship Solution,
based on an expansive study of 180 U.S. hospitals through its Antimicrobial
Stewardship Pilot Program.

Polk’s Drugs is a small chain of pharmacies based in
Mississippi. The Ocean Springs store, which opened
about a year ago, is unique in that it also includes a
gas station.
For manager Jesse Murphy, PharmD, choosing

SOLUTIONS: In January 2018, the Compliance and
Regulatory Services team provided a full assessment
of compounding safety practices from procurement
to administration in Dayton Children’s Hospital’s four
compounding locations that included review of:

Workflow observations

Equipment and engineering controls

Environmental sampling

Cleaning and disinfection

Policy and procedures surrounding sterile
compounding
Following the assessment, the Compliance and Regulatory
Services team drafted a Compliance Report and developed
a plan for process improvement designed to optimize
safety, efficiency and effectiveness, and compliance.
At the request of the organization, due to a plethora of
ongoing projects on the current Director of Pharmacy’s
agenda, the Compliance and Regulatory Services team
offered a custom approach to managing the execution of
the project plan remotely. Currently an ongoing project,
the Compliance and Regulatory Services team is virtually
managing implementation and execution of the line items
in the plan, ensuring that timelines are met and freeing up
the Director of Pharmacy for other responsibilities.
RESULTS: As of March 2018, the project plan is approaching
25 percent completion. In February 2018, the Board of
Pharmacy surveyed one of the organization’s four sterile
compounding locations. Because some of the items identified
in the project plan were being acted upon, the surveyor issued
no further citations. At another location, guidance is currently
being provided for a full remodel of the sterile compounding
suite which includes working directly with the pharmacy team,
architects, engineers and facility administration.
THE CHALLENGE:
Achieving and sustaining compliance across the entire healthcare organization and lowering regulatory
risk while improving operational efficiency and effectiveness are critical goals for hospital and health
system pharmacies. When Dayton Children’s Hospital reached out to Comprehensive Pharmacy Services
(CPS) for support in assessing compliance and regulatory issues for controlled substances, sterile
compounding (USP <797> and <800>), The Joint Commission standards and 340B, CPS responded
with custom solutions from its Compliance and Regulatory Services and 340B teams.
